About the Reablement and Care Support Worker Role

At its heart, this role is about helping people stay in their own homes and live as independently as possible. Reablement support workers work directly with individuals in the community, implementing Personal Plans designed to help service users regain skills, build confidence, and maintain the quality of life they deserve.

The work is frontline, hands-on, and genuinely person-centred. Every day looks different — and every day, the work you do has a direct, visible impact on a real person’s life.

This role is registered with the Care Inspectorate for Wales under the Registration and Inspection of Social Care Act 2016, meaning all practice is carried out to a recognised national standard of professional care.

What You Will Do Day to Day

Community-Based Front Line Care

The primary responsibility is providing front-line care and support to individuals living in their own homes across the Welshpool area. Every interaction is guided by a person-centred approach — meaning the focus is always on what the individual needs and wants, not a one-size-fits-all model.

Promoting Independence

A key part of the role involves actively supporting individuals to achieve their personal outcomes and regain as much independence as possible. This is not passive care — it is purposeful, goal-driven support that gives people back control over their own lives.

Working With Other Professionals

Reablement workers do not operate in isolation. The role involves close collaboration with colleagues and other professionals to promote well-being, take measured and positive risks, and build community presence for the people being supported.

Two Contract Options

This position carries a requirement for an Enhanced DBS (Disclosure and Barring Service) check, which is standard for all roles involving vulnerable adults. The council will guide successful applicants through this process.
